    The Burlingame Police Department maintains a confidential database of emergency contact information to help us better respond to your business when there is an alarm or other problem. All information is strictly confidential and used only for emergencies such as a burglary, a fire or an alarm. Filling out this form will ensure that the Police and Fire Departments have the proper contact information for your business.

    We urge you to download, print and complete the applicable forms. We recommend that you list at least two emergency contacts, those that have access to your building, and who can respond rapidly in the event of an emergency. Please include a second means of contact, such as cell phone or pager numbers. All information will be verified. The completed form can be faxed to 650-697-8130 or mailed to the Burlingame Police Department.
